The partial removal of the oesophagus and stomach with construction of a gastric conduit.

Oesophageal cancer—after multidisciplinary team (MDT) discussion and complete staging (CT chest, abdomen, and pelvis, staging laparoscopy, positron emission tomography (PET), endoscopic ultrasound).

Severe strictures associated with reflux, lye ingestion, or end-stage achalasia.

Para-oesophageal hernia.

Gastric cancer involving the gastro-oesophageal junction.

CONTRAINDICATIONS

Oesophageal cancer T4b staged lesions—with invasion of the trachea, aorta, spine, or other crucial structures or with distant metastases and thus inoperable.

Where the stomach is diseased or previously removed; other reconstructive options would need to be considered (e.g. colonic interposition).

Those with significant comorbidities, poor performance status, or severe malnutrition.

Poor lung function that precludes one lung ventilation.
