In den vergangenen Jahren hat die Neuroonkologie eine tiefgreifende Entwicklung vollzogen. Das Verständnis der Biologie vieler Tumoren ist durch die Ergebnisse der Molekularbiologie und -genetik wesentlich differenzierter geworden. So konnten durch umfangreiche Studienserien biologisch und klinisch distinkte Subgruppen identifiziert werden, so zum Beispiel bei den Medulloblastomen und den Gliomen im Erwachsenenalter. Der allgemeine Teil des Manuals stellt Grundzüge der Diagnostik und Therapie dar. Im speziellen Teil werden die einzelnen Tumorentitäten in Hinblick auf Häufigkeit, Prognose und Therapie fokussiert. Die Rehabilitation wird in einem eigenen Kapitel behandelt, ebenso wie palliativmedizinische Maßnahmen. Neben der inhaltlichen Überarbeitung und Aktualisierung wurden in der 4. Auflage des Manuals zu Beginn jedes Kapitels "Abstracts" eingeführt. "Essentials" heben die Kernaussagen einzelner Abschnitte hervor.
