Das multiple Myelom ist ein vom Knochenmark ausgehender, relativ seltener Tumor. In den vergangenen Jahren hat sich Vieles in der Diagnostik und Therapie verbessert. Neue therapeutische Möglichkeiten haben zu Zulassungen von weiteren Medikamenten geführt – neuen Proteasominhibitoren, immunmodulatorischen Substanzen und monoklonalen Antikörpern.Die autologe Blutstammzelltransplantation bleibt jedoch ein zentrales Therapieelement, auch viele ältere Patienten können hiervon profitieren. In Kombination mit neuen Substanzen, gegeben im Rahmen der Induktion vor und als Konsolidierung oder Erhaltung nach der Transplantation, werden gute Ergebnisse in der Erstlinientherapie erzielt.Mit den 2014 überarbeiteten Kriterien für die Diagnose eines multiplen Myeloms wird angestrebt, für Patienten, die ein hohes Risiko für die Entwicklung klinisch relevanter Organschäden haben, ohne dass diese bereits vorliegen müssen, eine rechtzeitige Therapieeinleitung zu gewährleisten – vor Eintritt der Schäden. Die Diagnostik und die Definition der Tiefe einer Remission nach Therapie entwickeln sich weiter. Bei immer mehr Patienten wird durch die Therapie eine Komplettremission der Erkrankung erreicht. Dennoch treten Rezidive regelhaft zu unterschiedlichen Zeitpunkten auf. Die Ursache hierfür ist die sogenannte "minimal residual disease", die durch hochsensitive Methoden erfasst werden kann. Das Manual befasst sich sehr ausführlich mit all diesen Themen. Es wurde von einem interdisziplinären Autorenteam erarbeitet und ist als Handbuch für die klinische Praxis gedacht, für Ärztinnen und Ärzte, die Myelompatienten betreuen.
