The Miracle of the Human Brain—The Basis for Neuropsychological Rehabilitation

We are all well familiar with the touching story of Paul Bach-y-Rita, one of the most influential theorists of neural plasticity. It is well known that Paul Bach-y-Rita was greatly influenced by his father Pedro’s miraculous recovery following a severely debilitating stroke. Paul’s brother George Bach-y-Rita employed some very unconventional methods to rehabilitate his father and succeeded well beyond anybody’s imagination. Pedro Bach-y-Rita recovered enough to take on full-time teaching and died 7 years after a heart attack in a mountain-climbing expedition. The story was far from over with Pedro’s death. Postautopsy reports indicate that approximately 97% of the cerebrospinal nerves were destroyed. Needless to say, this was a huge blow to the localization theory, which states that brain structures are hardwired to perform a limited set of functions.
The enchantment of neuropsychological rehabilitation (NR), therefore, rightfully lies in the marvel of the brain itself. The brain is an organ that continues to remain an enigma. While the mysteries of the brain are slowly but surely being unravelled by advances in cognitive neurosciences, one can argue that we are no closer to the absolute truth.

Definition and Introduction

NR is concerned with the amelioration of cognitive, emotional, psychosocial, and behavioral deficits caused by an insult to the brain [1]. NR, needless to say, has made its mark as a vital aspect of intervention for brain-related dysfunction. At the outset, it must be stated that the authors do not believe in the existence of a standard NR procedure. While this particular statement made may seem debatable, maybe even controversial, the need for it to be stated is that the authors take on a purely phenomenological approach to rehabilitation. While it is generally agreed that there are certain basic principles of rehabilitation, the manner of rehabilitation depends on several factors. The course of intervention is decided based not only on the diagnosis of the patient but also on the individual patient’s age, premorbid functioning, education, background, etc., and the therapist’s innovative skill in developing tasks to address these functions. Also to be kept in mind are the behavioral, social, and emotional ramifications of the dysfunction in the patient’s life as well as in the lives of those interacting with the patient.
NR has application in several brain-related conditions such as traumatic brain injuries (TBIs), tumors, cerebrovascular accidents, anoxia, brain-related infectious conditions, neurodegenerative conditions, and psychiatric disorders. The nature of brain-related pathophysiology is different in these various conditions, and the thrust of the rehabilitation procedures needs to be modified accordingly. Although the basic outlook of rehabilitation in these conditions tends to be varied, the basic principles of NR remain constant. However, it must be stated that among all these conditions, it is the area of TBI rehabilitation that appears to be most well researched. The reasons for this are yet unclear. Possible explanations may include the extreme burden of health care, loss of productive years of the individual, and loss of manpower resources that such injuries entail. It may also be due to the increasing incidence of such injuries in adolescents and young adults. In India alone, TBI has an incidence of 150/100,000 and a mortality rate of 20/100,000 [2].
While cognitive dysfunction forms the core aspect of intervention, a neuropsychologist’s role is not simply limited to the remediation of cognitive deficits. NR is also interested in addressing the emotional disturbances associated with the disorder, behavioral dysfunctions, and the lacuna in social functioning subsequent to brain dysfunction.

Neuropsychological Assessment—A Precursor to the Development of a Sound Neuropsychological Rehabilitation Program

The importance of neuropsychological assessment in planning intervention modules can never be overemphasized. Neuropsychological assessment is an interface between neurology, psychiatry, and psychology, and it represents various convergent disciplines, including neuroanatomy, neurochemistry, neurophysiology, cognitive neuroscience, and neuropharmachology. Neuropsychological assessment measures encompass a wide range of tests, beginning with 10-minute screening tools and extending to exhaustive batteries that last for a few hours. Both qualitative and quantitative measures form an important part of neuropsychological assessment. Significant aspects of neuropsychological testing include assessing the mental state of the individual and testing the functions of the brain based on its known physiology. Neuropsychological tests are scientific, standardized on normal individuals. Tests have been designed to assess sensory input, attention and concentration, learning and memory, language functions, spatial and manipulatory ability, executive functions, and motor output [3].
The neuropsychological assessment provides diagnostic information about the nature and extent of cognitive dysfunction following various neurological, neurosurgical, and psychiatric conditions. In addition to its utility as a diagnostic tool, assessment findings play a vital role in planning NR. Hence, the aim of cognitive rehabilitation is to address cognitive dysfunction. Apart from this, neuropsychological intervention also attempts to address the emotional, social, and psychological implications of brain pathology.
Several cognitive deficits have been associated with TBI. Cicerone [4] reports deficits in attention and difficulty in dual task demands even with patients with mild TBI. McDowell et al. [5] confirm working memory deficits in individuals with TBI using other dual task paradigms. Brooks et al. [6] report significant differences in frontal lobe executive functions in individuals with TBI in comparison with control subjects. Difficulties in memory have also been reported. In a study by Hamm et al. [7], deficits in performance on the Morris water maze was found to be diminished in rats that had sustained brain injury.
Other aspects of neuropsychological functioning include assessment of stress, depression, anxiety, behavior analyses, social functioning, and quality of life. Addressing these factors helps in assessing outcome and enables better prognostication. A well-administered neuropsychological assessment not only prognosticates but also helps in the development of specific goals for rehabilitation.

Theories of Neuropsychological Intervention

A good NR program requires a foundation of strong theoretical frameworks.

The Role of Neuronal Plasticity

The environment can alter the organization of the normal and injured brain through the mechanism of brain plasticity [8]. Neuronal plasticity refers to the adaptation of the brain as a result of several factors that may independently or through interaction produce changes in the brain. Neural plasticity may occur as a part of normal developmental function, or it may be the result of a response to destroyed brain function due to brain injury. The environment plays an important role in brain plasticity. Enriched environments have been demonstrated to be associated with greater levels of dendritic growth, particularly in the hippocampus. This ability of the human brain to adapt itself, is what makes neurorehabilitation possible.

The Role of Restitution, Substitution, and Compensation

In 1947, Zangwill (as cited in Wilson and Glisky [9]) described three processes of rehabilitation. The first, “restitution,” describes a process of restoration of the lost or impaired function. The second, referred to as “substitution training,” involves the replacement of impaired functions by alternate strategies that prove to be functional. This may also involve the reorganization of cortical networks associated with the task. The third, which is described as “compensation,” requires an artificial source that typically requires more effort and time.

The Role of Diaschisis and the Unmasking of Latent Neural Pathways

Diaschisis refers to the excitatory and inhibitory effects following brain injury that affect areas remote to the site of injury. These effects were used to explain the sequelae following the injury. Although diaschisis was originally used to explain the deficits following head injury, it may indeed work as a blessing in disguise. In a normal uninjured brain, there exist certain pathways that are ineffective and inactive. Diaschisis may indeed inhibit this original suppression thereby rendering these alternate pathways functional [10].

Major Influences and Recent Advances in the Development of Rehabilitative Neuropsychology

To begin our understanding of the present need and scope of NR, it is necessary to frame the current trends in the perspective of the past of NR and where its current practices stem from.
The earliest recorded information we possess on localization of function in the brain is contained in the Edwin Smith Surgical Papyrus. The copy acquired by Smith in Luxor in 1862 is thought to date from the seventeenth century BC, while orthographic and other evidence would place its origin some thousand years earlier, somewhere between 2500 and 3000 BC. It contains the earliest known anatomical, physiological, and pathological descriptions and has been described as the earliest known scientific document. The papyrus contains reports of some 48 cases of observation and description of treatment of actual cases, many of them suffering from traumatic lesions of various parts of the body, including the head and the neck. Of the 48 cases, the first 8 deals directly with injuries to the head and the brain [11].
Other important influences include the proposition of the Hebbian law that succinctly states that “what fires together wires together.” The original theory reads as follows: “When an axon of cell A is near enough to excite cell B or repeatedly or persistently takes part in firing it, some growth process or metabolic change takes place in one or both cells such that A’s efficiency, as one of the cells firing B, is increased.” The activity of one neuron that influences another neuron thereby forms the basis of neuronal plasticity through long-term potentiation and long-term depression.
Attempts at rehabilitation can be traced back to Paul Broca’s attempt to enable a patient who had reading difficulty [12]. Efforts at rehabilitation can also be attributed to Shepherd Franz, who was known for his work on motor learning in patients with hemiparesis and therapeutic approach with aphasic patients. The centers for rehabilitation set up in Europe during the world wars also played a vital role in unfolding of the rehabilitation as we understand it now. Goldstein’s documentation of treatment of disorders of speech, reading, and writing, the effects of fatigue on performance, and the unique manner in which each patient is to be treated also deserve a special mention [12]. The methods of assessment during the time of World War II resulting in a formal detailed neuropsychological profile can be credited to Russian psychologist Luria. The work of Ben-Yishay in the late 1970s and the 1980s also highlighted the importance of considering the interpersonal and social aspects of the patient in fostering reintegration into the social world [13].
Of course, we have progressed much since the Edwin Papyrus. Lesion and radiological studies have of course ascribed specific functions to specific locations. However, as mentioned earlier, the work by Bach-y-Rita and several others indicate that this is not so simple.

Methods of Neuropsychological Rehabilitation

There has been a change in the perspective of intervention in the care of brain-related injuries. While initial perspectives focused on acute management and intensive medical care targeting basic survival, later approaches appear to address the outcomes of the injury in terms of basic functioning and quality of life.
