According to the National Institute of Diabetes and Kidney Digestive Diseases, 60 million people experience heartburn at least once a month and 25 million experience symptoms daily.
Gastroesophageal Reflux Disease (GERD), a more serious form of acid reflux, is the most common digestive disorder (Lock et al, 1997) in the United States. Studies (Dent et al, 2005) show that 10-20% of individuals’ experience symptoms at least once a week, and prevalence of GERD is increasing steadily.
As sobering as those statistics are, it’s likely that the prevalence of GERD is underestimated because of the availability of over-the-counter antacids. These permit patients to self-medicate without reporting their condition to a doctor.
Up until recently, heartburn wasn’t taken too seriously, but we now know that heartburn and GERD can have serious and even life-threatening complications, including scarring, constriction, ulceration, and ultimately, cancer of the oesophagus.
Recent studies also show that the damage from poor stomach function and GERD not only extends upward to the sensitive oesophageal lining, but also downward through the digestive tract, contributing to Irritable Bowel Syndrome (IBS) and other gastrointestinal problems. IBS is now the second-leading cause of absenteeism at work, behind only the common cold.

Understanding the Oesophagus and Stomach
When we eat, food passes down the gullet (oesophagus) into the stomach. Cells in the lining of the stomach called parietal cells produce hydrochloric acid (HCl) and intrinsic factor. HCl is responsible for the high acidity (pH 1.5 to 3.5) of the stomach contents and is needed to activate the protein-digesting enzyme, pepsin. The acidity also kills much of the bacteria you ingest with food and helps to denature proteins, making them more available for enzymatic digestion. Intrinsic factor is a glycoprotein necessary for the absorption of vitamin B12 in the small intestine.
Normally, a ring of muscle at the bottom of the oesophagus, called the lower oesophageal sphincter, prevents reflux (or backing up) of acid. This sphincter relaxes during swallowing to allow food to pass. It then tightens to prevent flow in the opposite direction.
With GERD, however, the sphincter relaxes between swallows, allowing stomach contents (gastric reflux) and corrosive acid to well up and damage the lining of the oesophagus.
What is Reflux and Oesophagitis?
Acid reflux means that some acid leaks up (refluxes) into the gullet (oesophagus).
Oesophagitis means inflammation of the lining of the oesophagus. Most cases of oesophagitis are due to reflux of stomach acid which irritates the inside lining of the oesophagus.
The lining of the oesophagus can cope with a certain amount of acid. However, it is more sensitive to acid in some people. Therefore, some people develop symptoms with only a small amount of reflux. Whereas some people have a lot of reflux without developing oesophagitis or symptoms.

Is Acid Reflux the same as GERD?
Acid reflux and gastroesophageal reflux disease (GERD; sometimes known as GORD in the UK as esophagus is spelt oesophagus), are closely related, but the terms don't necessarily mean the same thing. In this book, we will call it GERD.
Acid reflux is the backward flow of stomach acid into the oesophagus - the tube that connects the throat and stomach. Acid reflux is more specifically known as gastroesophageal reflux or GERD for short. During an episode of acid reflux, you may taste regurgitated food or sour liquid at the back of your mouth or feel a burning sensation in your chest (heartburn).
Sometimes acid reflux progresses to GERD, a more severe form of reflux. The most common symptom of GERD is frequent heartburn. Other signs and symptoms may include regurgitation of food or sour liquid, difficulty swallowing, coughing, wheezing, and chest pain - especially while lying down at night.
Severe chest pain develops in some cases and may be mistaken for a heart attack.
Symptoms of Acid Reflux (GERD)?
Persistent heartburn is the most common symptom of GERD.
  • Heartburn is a burning pain in the centre of the chest, behind the breastbone. It often starts in the upper abdomen and spreads up into the neck or throat
  • The pain can last for as long as 2 hours
  • Heartburn is usually worse after eating
  • Lying down or bending over can bring on heartburn or make it worse
  • The pain usually does not start or get worse with physical activity
  • Heartburn is sometimes referred to as acid indigestion
  • Not everyone with GERD has heartburn
Other symptoms of GERD include the following:
  • Regurgitation of bitter acid up into the throat while sleeping or bending over
  • Bitter taste in the mouth with possible bad breath and gum problems
  • Persistent dry cough. This is due to the refluxed acid irritating the windpipe (trachea). Asthma symptoms of cough and wheeze can sometimes be due to acid leaking up (reflux)
