BASIC CHARACTERISTICS

Class: Alpha interferon.
Mechanism of Action: After binding to the cell‐surface receptor, production of several interferon‐stimulated gene products lead to antiviral, antiproliferative, and immunomodulatory effects, regulation of cell surface major histocompatibility antigen (HLA class I and class II) expression, and regulation of cytokine expression.
Mechanism of Resistance: Unknown.
Metabolic Route: Interferon is metabolized into amino acids.

FDA‐APPROVED INDICATIONS

Both Infergen and Intron A are indicated for treatment of chronic hepatitis C in patients 18 years and older.
Intron A is also indicated for the treatment of hepatitis B.
Intron A is also indicated for hairy cell leukemia, malignant melanoma, follicular lymphoma, condylomata acuminata, and AIDS‐related Kaposi sarcoma.
Other uses: Hepatitis B.

SIDE EFFECTS/TOXICITY

WARNING: Alpha interferons cause or aggravate fatal or life‐threatening neuropsychiatric, autoimmune, ischemic, and infectious disorders. Patients should be monitored closely with periodic clinical and laboratory evaluations. Patients with persistently severe or worsening signs or symptoms of these conditions should be withdrawn from therapy. In many but not all cases these disorders resolve after stopping alpha interferon therapy.
Interferon is contraindicated in patients with known hypersensitivity to alpha interferons or to any component of the product, decompensated hepatic disease, or autoimmune hepatitis.
Other adverse events:
General: fever, hypersensitivity.
Neuropsychiatric disorders: depression, suicide, psychosis, aggressive behavior, nervousness, anxiety, emotional lability, abnormal thinking, agitation, apathy, and relapse of drug addiction.
Infections
Bone marrow suppression
Cardiovascular disorders, including hypertension, tachycardia, palpitation, tachyarrhythmias, supraventricular arrhythmias, chest pain, and myocardial infarction.
Hypersensitivity
Endocrine disorders: hyperthyroidism, hypothyroidism, hyperglycemia, diabetes mellitus, elevated serum triglycerides.
Autoimmune disorders: autoimmune thrombocytopenia, idiopathic thrombocytopenic purpura, psoriasis, SLE, thyroiditis, and rheumatoid arthritis.
Pulmonary: pneumonia and interstitial pneumonitis.
Gastrointestinal disorders: hemorrhagic/ischemic, ulcerative colitis, pancreatitis, and hepatic decompensation.
Ophthalmologic disorders: decrease or loss of vision, macular edema, retinal artery or vein thrombosis, retinal hemorrhages, cotton wool spots, optic neuritis, and papilledema.
Cerebrovascular disorders: ischemic and hemorrhagic cerebrovascular events.

DRUG INTERACTIONS/FOOD INTERACTIONS

DOSING

Chronic hepatitis C
Infergen (interferon alfacon‐1): nine mcg three times weekly administered subcutaneously as a single injection for 24 weeks. At least 48 hours should elapse between doses.
Patients who did not respond or relapsed following its discontinuation may be subsequently treated with 15 mcg times a week (tiw) administered SC as a single injection for up to 48 weeks.
Dose reduction to 7.5 mcg may be necessary following an intolerable adverse event.
Intron A: 3 MIU three times a week (tiw) administered subcutaneously or intramuscularly for 18–24 months in patients who tolerate therapy and normalize ALT at 16 weeks of treatment. If severe adverse reactions develop during Intron A treatment, the dose should be modified (50 % reduction) or therapy should be temporarily discontinued until the adverse reactions abate.
Chronic hepatitis B
Intron A: 30 to 35 million IU per week, administered subcutaneously or intramuscularly, either as 5 million IU daily (q d) or as 10 million IU three times a week (tiw) for 16 weeks.
